Based on the findings of an off-site state relicensure survey completed February 21, 2023, Holland Home Care LLC, was found to be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51.

Based on the findings of an off-site state relicensure survey completed February 21, 2023, Holland Home Care LLC, was found NOT to be in compliance with the following requirement of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries.

(c) Prior to the commencement of services, the home care agency or home care registry shall provide to the consumer, the consumer's legal representative or responsible family member an information packet containing the following information in a form that is easily read and understood: (1) A listing of the available home care services that will be provided to the consumer by the direct care worker and the identity of the direct care worker who will provide the services. (2) The hours when those services will be provided. (3) Fees and total costs for those services on an hourly or weekly basis. (4) Who to contact at the Department for information about licensure requirements for a home care agency or home care registry and for compliance information about a particular home care agency or home care registry. (5) The Department's complaint Hot Line (1-800-254-5164) and the telephone number of the Ombudsman Program located with the local Area Agency on Aging (AAA). (6) The hiring and competency requirements applicable to direct care workers employed by the home care agency or referred by the home care registry. (7) A disclosure, in a format to be published by the Department in the Pennsylvania Bulletin by February 10, 2010, addressing the employee or independent contractor status of the direct care worker providing services to the consumer, and the resultant respective tax and insurance obligations and other responsibilities of the consumer and the home care agency or home care registry.

Observations:


Based on review of the home care agency's admission packet given to consumers, consumer files and staff interview, it was determined the home care agency failed to identify the Department's complaint hotline number in a form that is easily read and understood for 11 of 11 consumer files reviewed. (CF1, CF2, CF3, CF4, CF5, CF6, CF7, CF8, CF9, CF10, and CF11).

Findings include:

On December 9, 2018 at approximately 12:15 p.m., a review of the agency's admission packet revealed that page 15 in the admission packet listed the Department of Health's general number, but failed to label Department's "Complaint Hotline," as the number, in a form that is easily read and understood . The agency last documented the "Complaint Hotline," number incorrectly as 1-8660-826-3466. (The correct Complaint Hotline number is 1-800-254-5164).

1. On February 21, 2023, at Approximately 12:20 p.m., a review of CF #1 revealed a start of service on November 28, 2022. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

2. On February 21, 2023, at Approximately 12:25 p.m., a review of CF #2 revealed a start of service on September 19, 2021. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

3. On February 21, 2023, at Approximately 12:30 p.m., a review of CF #3 revealed a start of service on August 12, 2019. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

4. On February 21, 2023, at Approximately 12:35 p.m., a review of CF #4 revealed a start of service on January 20, 2023. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

5. On February 21, 2023, at Approximately 12:40 p.m., a review of CF #5 revealed a start of service on July 18, 2019. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

6. On February 21, 2023, at Approximately 12:45 p.m., a review of CF #6 revealed a start of service on January 24, 2021. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

7. On February 21, 2023, at Approximately 12:50 p.m., a review of CF #7 revealed a start of service on December 16, 2021. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

8. On February 21, 2023, at Approximately 12:55 p.m., a review of CF #8 revealed a start of service on April 1, 2022. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

9. On February 21, 2023, at Approximately 1:00 p.m., a review of CF #9 revealed a start of service on May 5, 2022. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

10. On February 21, 2023, at Approximately 1:05 p.m., a review of CF #10 revealed a start of service on January 1, 2019. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

11. On February 21, 2023, at Approximately 1:10 p.m., a review of CF #11 revealed a start of service on November 1, 2019. There was no evidence that the Compliant Hotline number was written in a form that was easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

In an interview with the administrator, on February 23, 2023, at approximately 8:21 a.m., an email was sent to confirmed the findings that the Compliant Hotline number was written in a form that was not easily read and understood by the Consumer, a Power of Attorney, and/or a Responsible Party.

Holland Home Care Will Correct/Replace current incorrect Department of Health Hotline Number
Holland Home Care will discard all admission packets with incorrect information and replace them with corrected material.
Holland Home Care will inform all consumers via USPS Mail service of error in Admission packet information. HHC will include correct Department of Health Hotline number in above stated latter.
Holland Home Care will also follow up with all current consumers to inform them of the correct DOH Hotline number.
Holland Home Care will update Current (incorrect) Hotline number to Correct Hotline Number:
1-800-254-5164

Based on the findings of an off-site state relicensure survey completed February 21, 2023, Holland Home Care LLC, was found to be in compliance with the requirements of 35 P.S. 448.809 (b).
